What to Bring

To make the most of your visit, please bring the following items to your appointment:

• Insurance card(s)
• List of current medications and dosages
• Any pertinent medical records, endoscopy reports, lab or x-ray results
• Your co-pay (the office accepts cash, checks, VISA and MasterCard)

New Patients

You can save time by printing and completing the new patient paperwork prior to their first visit. Please click on the following documents: new patient form and insurance form.

Office Hours & Appointments

The office of Southwest Digestive Specialists is open:

• Scottsdale – Monday – Friday from 8 a.m. – 5 p.m.
• Fountain Hills – Tuesdays (twice monthly) from 1 – 5 p.m.

Appointments can be made during that time, except between noon – 1 p.m., by calling 480-767-7273. To prevent being billed, office visit and procedure cancellations must be called into the office at least 24 hours in advance.

Privacy

Your privacy is considered a priority at Southwest Digestive Specialists. Our procedures and software programs meet or exceed the HIPAA standards required for physician practices. Please let us know if you have any questions.
